91 240 won't pass CT Emissions Test 200 1991

Hi my 1991 240 Wagon with 181,000 miles has failed twice to pass the state of Connecticut (USA) Emissions test.

The car seem's to be running slightly rich and even after changing the O2 sensor, which did help some with making it run cleaner and it did bring up up the test results on the second test. Other than that when she is running she runs smoothly, responds well to the accelorator and has no signs of running poorly.

I was wondering if it could be something as simple as changing the distributor cap and rotor, or is it possibly a leaking vacume hose or possibly the Air Mass Sensor, which was replaced around 6 years ago? Or am I looking in the wrong areas?

I also changed the exaust system last year after the old one rusted out.

Now, even with a new exaust and o2 I still can not get the car to pass emissions.

Any sugestions?
