question of radiator 200

This morning, wehn I tried to fix the whinshield washer , I found the coolant reservioer is empty.

After put some coolant into it ,I found the coolant leaked out from one hole(number 2) loacted in the right top of the radiator.

The radiator was replaced by a shop 5 years before.
It is SPECTRA PREMIUM Part # CU83 {#83}.

What it should be(open VS close)?
If I need to buy a cap to close it,Where to buy?
