Fishtail in a hard brake. 120-130 1968

Not sure if this is a mis-adjustment, worn pads, or something more serious.

My 1968 still sports a split brake system front/rear with a single brake limiter. Both are power boosted with a 140 brake system.

When I do a hard brake, the car invariably fishtails to the right and my right tail swings out. It is especially bad on wet asphalt.

I drive as defensively as possibly, but occasionally, I have to jam my pedal down to avoid being crushed by a large truck.

I have the self-adjusting rear brake shoes and I've had my hubs off for service, cleaning and adjustment.

What are the likely causes of this sort of problem?
