door lock stays "up' - lock mechanism (and door panels) 700

I have a 1991 745 with about 110k miles. It's a nice car with a few quirks. The most recently developed quirk is that happens when locking the car with the key in the driver's door. The door behind the driver will not actuate the locking mechanism; it remains in the up position.

What could be the cause?

I'm not looking forward to removing the door panel, as this car has nice, tight door panels and they never seem to go back on that way.
