A mechanics BS or actually possible? (not volvo related but interesting) 200

My brother has a dodge mid sized truck, 90 something with about 200,000 miles on it. The mechanic wants to replace the engine because two cylinders are not running because of bad intake valves. I have to question how two cylinders stop at the same time due to bad valves. Is this really remotely possible. He claims that if he replaced the valves the rest of them would only follow not too long after so its better to replace the engine. Sounds fishy to me. The truck smokes black a little when it first starts but has had plenty of power.

My brother trusts the mechanic but after all the stories I've heard on here about bad mechanics I have to doubt this one. From what I've heard he's replaced the cap and rotor with no help. My brother is struggling to pay his mortgage and doesn't need to be taken for a ride with this.

Sorry to bring a non 240/volvo situation but I figured there would be some honest opinions on here.
