Electrical Problem - Voltage Meter wont go higher than 12 volts 200

My 91 240 has issues. The car has 219K miles on the clock.
It all started with a dead battery.
I replaced the brushes and installed a New battery. First day of driving it, 15 miles later ...the battery was dead again.

I put in a New (rebuilt) alternator and fully charged the battery.
I then put in a voltage meter to see what the battery/charing system was doing and low and behold...the volt meter reads 12 volts and no higher even when on fast idle, when I put the lights on, the volts drop down to about 11.75.

Whats going on. I know in no time flat the battery will be dead again.
Whats going on with the charging system. I will say when it comes to the charging system...I'm a charging system illiterate. Can you guys help get this car up and running?
