All that, and more.

I record all my gas fillups (not the $, since I can't control that) and keep notes about where I have the cam timed, which octane I'm using, etc. It all goes into a master spreadsheet along with maintenance records.

Due to seasonal use of AC (6 months of the year in Phoenix area) and seasonal changes in gas formulations, I look at both overall gas mileage by fillup as well as averages by month over several years. The biggest difference I've found so far was when I put in a new oxygen sensor a couple of years ago.
