Cold Engine Hesitates, Dies 200 1987

The car is a 1987 240DL. When I start form cold, the car struggles to start. I have to keep cranking continuously until it starts. Then once the engine starts, it hesitates when pushing on the gas and stalls. It will only go if I push the gas very gently.
In the mean time I can hear a high pitch noise coming from the engine. Later when the car is warmed up the hesitation goes away, not the sound. Also a bit of smoke coming from the muffler. Someone, please, can guide me on these issues?. I am thinking on buying this car as a back vehicle.
