245 doesn't like to wake up 200 1981

I have an 81 245 that I'mhaving trouble with. The car starts immediately, but unless I let it sit and warm up for a few minutes, there's no going over 5 mph. Doesn't matter how far I press the gas pedal it runs at abouta fast walk. Once it's warmed up, all is good. What should I look at? The only other problem I have with this car is that when in stop and go traffic the idle drops to about 500 and has a surge.
