Weird problem 200

88 244, automatic. I'll try and describe the problem as best I can, so bear with me. Only happens when car is cold. After warm up, it goes away. Lights are slightly dim as well as dash lights. When motor is slowly revved, I hear a "clunk" and lights brighten up. I can not tell where the "clunk" is coming from. Today, put a meter on battery before start-up, read 12.8V. Started car, same voltage. Slowly revved engine until "clunk" and voltage went to 14.5V. It only does this when first driving the car and goes away after warm-up. As a side note, thought I could feel the steering wheel jerk very slightly when turning and during clunk. Don't know how this could be related at all to the voltage. Any thoughts, ideas, or other tests would be appreciated.
