Volvo 960 jerking around! 900

I have been experiencing a problem with the car jerking severely at about 60mph or around 25-2700RPM. It seems to do this lurching while I steady or hold the accelerator pedal or back off from accelerating at this speed. It is almost like it can't decide wheteher to shift into 5th or stay in 4th. You casn actually hear the shifting back and forth. It stops as soon as I start accelerating again or back off all together, any change to the pedal. Is there a shift sensor or something that needs replacing. It does shift through the gears well so I don't belive it is a tranny problem.
