245 rough idle when warm 200 1993

My wagon will start fine and idle perfect while cold, but once the engine comes up to temp, I get a rough, surging idle. Vacuum gauge indicates a pull, and it will remain as such for about 30 seconds, then the idle will smooth out. There is a slight lag in accelaration at first, then fine afterwards.

I had this issue before about 2 years ago, so I cleaned the throttle body and IAC and that helped a bit. It wasn't totally resolved until the EGR was disconnected. Recent smog necessitated that I hook it up back up, and of course, the problem has come back.

Recent work:
Full tune up- dizzy, cap, rotor, plugs, etc.
new O2 sensor
patched a crack in the accordion hose from the air box
