spark plug ceramic tip in cylinder 700 1992

Hey All,
Pulled #4 plug and surprised to see the white ceramic tip missing - thought it must have been for a while because it had been running a little rough. Put in new plugs and when I started it sounded awful. Only let it run a couple of seconds but it sounded like bent valves, knocking rods, I don't know it was just bad. I tried fishing a hose connected to shop vac down but can't get anything out of that cylinder. Is the damage already done? Am I looking at removing the head? The tip must have fallen out as I was removing the plug is my guess.
