Cooling issues 900

Hello, About 3 weeks ago I had all of my cooling hoses replaced at the Volvo shop. Well tonight as my family and I were driving home suddenly I smelled coolant. Followed by the red light with a radiator and fan symbol . I immediately pulled off the road and turned my car off. The temp gauge never went hot. I opened the hood and found the upper radiator hose off the radiator. My coolant reservoir was empty. The fan ran for like forever. Then stopped. I wonder what would cause the hose to come off like that? Any ideas?
