A) Yes replace them all. I had a mechanic replace just 1 a few years ago after he told me I probably only needed one... and the others were shot too. Heh.
If one's gone, the others likely are too. Ditto on checking the center support bearing.

As for how to measure, when I ordered mine, it said "for 2-inch driveshaft" and "for 1.75-inch..." and so on. Do they not have an application guide like that? Supposedly, all 3 of mine are identical, but I haven't checked yet.
