Dash lights and alternator not charging 200

My '89 240 has the service, battery, parking brake, brake failure, & bulb failure lights lit up while running.

The battery light illuminates with the key on/engine off.

The alternator is not charging the battery, only 12 volts at the battery while running.

I've got a solid ground on the alternator, verified with a DVM.

I have 12.9 volts to B+ (thick red wire) but only 9 volts to D+ (thin red wire) with the key on/engine off.

It seems that D+ should be 12.9 volts but I don't know why the voltage is low.

Any ideas?
