It sounds like the intank pump is either not working and or the rubber tube between the pump and the sending unit is torn. It is not supplying th main pump with fuel particularly at less than 1/2 tank of fuel.

Also you may need to clean the throttle body, idle air control valve and flame trap (PCV system).
