OD arrow stay lit all the time & OD not engaging 200 1991

A friend of mine has a '91 245 with 145K miles on it. All was well with the auto transmission until one day the orange OD arrow stayed lit when the car was started. At idle, the arrow pulsates slightly and when given some gas, it stays on at full solid orange with no flickering. Put a know good white OD relay and same problem. Took it out for a drive and found the tranny won't go into 4th gear. Trans fluid level is ok. What do I check next?
