Alternator commutator wear 200 1992

When is an alternator commutator worn out? Can you tell by a visual inspection? Can the wear be measured as can brush wear? Any concerns with installing new brushes on "worn" commutators as opposed to "worn-out"?

Did not find these answers in FAQ or internet search.
