That depends on where you are. In Oregon, neither the DMV nor the insurance company cares what wheel size you have... nor where you got it. We used to have the local welder make stock wheels into 6 or 7 inch widths and no one ever asked if he was qualified to 'make' wheels. No testing, no certification, nothing. There's no state inspection to ensure that a given vehicle is roadworthy, even. The only testing you'll find is for smog, and that's only performed on cars built after 1977. Older cars are exempt. Makes it easy to run gross polluters on public roads, if that's what you're into.
Should you assemble a 1950s or 1960s car from leftover parts that should all have been melted down and recycled due to rust or wear or damage, and you're confident enough to drive it, neither the state nor the industry pays any heed. The insurance company only wants you to make your payments, and the DMV just wants you to make your payments.
If you cause a crash, the insurance will investigate with the hope that they can find themselves not liable. And because our medical coverage is privatized, the state doesn't care who injures who. Not exactly preventative nor proactive.
