Valve train noise. How to proceed.

MY 91 240(200,000 miles)has noise coming from the valve train area. It is not piston slap. Though the engine does have slap, this goes beyond that(it doesnt go away with warm up). Im going to be checking to see what the problem is soon. Should I plan on just fixing the obvious problems, or just replace everything? Im not sure what the problem is, but Im expecting, perhaps a broken valve spring. I dont know if a missing or worn husher would account for the noise.
