volvo 140 fuel pump 140-160 1968

hi, i replaced my fuel pump on my 1968 142s about 4 months ago and everything seemed fine for about two weeks, but then i noticed that the mounting bolts on fuel pump were loose so i tighten them up. the following week i checked the bolts again and they were loose once more. i also noticed that the diaphragm screws started to become loose too. i've been tightening bolts and screws for four months. now just today i noticed a puddle of oil under my car which is coming from the fuel pump. did i install the pump wrong? do i have a faulty pump? why are the mounting bolts and diaphragm screws coming loose? did i tighten the mounting bolts too tight? thank you for your help. regards, steve.
