upper radiator hose doesn't stay on 200 1993

Here is my story :

Check engine light came on, didn't see or feel any problem, kept driving.
Accelerating from a stop, there was a bang and a small amount of smoke from under the hood and the engine quit running.
Restarted easily, drove short distance.
Top radiator hose was disconnected from radiator. The hose was easily re-connected without loosening the clamp. How was the clamp holding the hose on ?
OB diagnostic codes were socket 2:123 and socket 6:224 for ECT sensor faulty
Re-connected hose, filled coolant reservoir.
Cleared codes.

Drove fine for several hundred miles,
then same 123, 224 codes,
and the fitting on the radiator that the upper hose clamps to was broken off and crumbling. Clamp too tight ?

I assume the radiator needs to be replaced because of the broken fitting but why is the hose getting blown off ?

Any suggestions will be welcomed.
