When the marks are together, both #1 and #4 are at TDC, #1 at the top of the exhaust stroke and #4 at the top of the compression stroke. The distributor gear orientation is shown as it should be when #1 is at the top of the compression stroke, so you have to turn the crank 360 degrees from the point where the marks are together.
