B18 With unidentified rattling noise

I'm posting for a friend who is going nuts trying to diagnose a rattling noise in his engine.
There is little power, acceleration is flat and slow.
The noise occurs when the engine is warmed up and at low idle.
There have been a couple of expert opinions, with the final diagnosis "Lets tear it down, probably going to need a rebuild"

Any ideas?


The engine has slightly lower compression on #3 cyl
Timing 12 deg BTDC
