1992 exhaust header with missing plug ... 200 1992

The missing bolt and washer plug a port used to analyze exhaust gasses before the catalytic convertor.

The plug you mention is near and before (upstream) of the header pipe out to the catalytic convertor in union?

If equipped with EGR, the first EGR pipe connects at the back of the manifold just above the manifold out to header pipe input union.

Not sure of the thread and thread pitch. You'll want some manner of high temp carbon steel or better, stainless, hex bolt. Apply nickel anti seize to the thread.

What I believe you are describing is the bolt at the end of the header pipe, at the top and to the left a bit of the triangular spin flange in this iPd image:
