Starter noise (Bendix or flywheel ring gear?) 200 92

The starter on my '92 240 makes a grinding noise about one out of every 3 starts. I can lower the frequency by backing off the key a little early. Two questions:

1) Is the problem usually chipped teeth on the flywheel ring gear or could the Bendix be at fault?

2)What are the rough repair costs of replacing the ring gear?

3) Are rebuilt starters reliable?
