T-belt stripped - again! 200 81

Well, my t-belt stripped on me again. Second time in 9 weeks. First time, it looked like the belt walked toward the timing cover and somehow got chewed up. I haven't yet taken apart the engine this time around (the car is 30 miles away from me - in north Seattle), so, don't know what's wrong this time. My thinking is that it is the same problem and that the crank gear somehow got damaged so that it wobbles on the crankshaft and causes the teeth on the belt to get munched up, or the gear may've been put on backwards. Happened about 2 hours after I finally got my 242 set up correctly in relation to the idle mixture, which, btw, was full-lean.
