High Rpms... whats up? 88 745

My wagon runs at 4000-5000 Rpms at 50 mph,,, and at 75 mph its 5000-5500,,, it sounds real rough,,, like its not getting into high gear... is transmission the problem? What do i do... it feels like the car is going to fall apart because of the vibration.
