Clutch judders on takeoff

I had my clutch replaced after 232k miles on the original. The clutch disk, pressure plate, throw-out bearing, and rear main oil seal were replaced. And the flywheel was resurfaced.

Now, just a few months later, I get a good amount of lurching and bucking when I start off in first gear from a stop. Oddly, it generally (but not always) goes away once the engine warms up. I can minimize the problem by slipping the clutch and giving it more gas. It don't have a problem in reverse gear at all.

Any ideas what's wrong?
