Blown FREEZE Plug 200 89

I just put the first 2000 miles on my rebuilt engine I did this summer.

I had an machine shop bore and replace the freeze plugs. I had noticed a small leak on one of my plugs earlier. The machine shop told me to get some radiator stop leak should take care of the problem. Last night driving back from business I blew the freeze plug towards the back on the intake side.

Can anyone help me with what may have gone wrong on the replacement of the Freeze plug. So I can try and get this fixed rite.
