There is no stop... I don't know what keeps it out besides pressure from the clutch assembly.

Hmm, yes, and manuals like hayne's will give you clearances.

That's exactly what happens. Our 240 is at the top of it's travel right now, with no more adjustment available, but this is with 264k miles.

Didn't you do the aw70-m46 conversion?? Well, there is no spring. The clutch prongs push the throwout bearing. I've heard it's bad to use the clutch as a foot rest as it engages the bearing and causes extra wear, but I think you have a point. How do you know that the bearing isn't in contact all of the time if nothing is holding it? I don't know.

I'd say in the middle... You don't want it near the top because the clutch may never fully disengage. However, you don't want it near the bottom because you may not be able to fully engage the clutch. Manuals will tell you specs you could follow... I didn't really follow them exactly, just used them as a rough idea. Just some distance before it starts to engage is what i though of it as. Some may argue with this, but I find everything works fine now.
