Engine maintenance 200 1987

I know that Volvo recommends adjusting valve clearances (every 80k?).

I’ve had my ’87 240 for past 14 years, with 215k and never had it done. What’ the consequences? Shorter engine life? Poor gas mileage? Should I do it? How much will it cost?

Another one: Timing belts replaced twice after they broke. Should I be proactive and replace it before they break?

I appreciate opinions on above.
