Tires installed 'inside-out' 700 1990

The directional tires I have can be mounted with either side in or out. All 4 tires are identical. I hadn't heard of tires that had "inside only" on them, but I guess that should make it easier to mount correctly, although requiring two different tires makes it more likely for the distributor to mess up. I had mine mounted 2 at a time, so the first pair could be mounted either way, but the second pair had to be mounted a certain way based on the first pair so I didn't end up with 3 or 4 for one side and only 1 or 0 for the other side.

If you had 4 directional tires installed on wheels and they're all going the same way, then you have 4 left-side-only tires or 4 right-side-only tires. Swapping sides does no good because 2 will always be correct and 2 will be wrong.

Regarding the full size spare, I'd probably leave the tire off the wheel and caryy a cheap spare tire. Then if the damaged tire just needs to be plugged, it can be put back on the car. If the tire destroyed, then mount the spare in the correct direction and put it on the car. In the case of the side-specific tires marked "inside only", then I don't see the point in having a full size spare. You'd need 1 for each side since they're different, so I'd probably use a cheap spare and just order a replacement when needed.
