Pulsing brakes, shaking steering wheel and shifter. 700 1992

I posted on here before about a problem with pulsing brakes and shaking steering wheel and shifter. I got some excellent responses but I have one more question. First, I'll relate the problem more clearly.

The steering wheel shakes and the whole car vibrates noticably above ~50 mph. The brake pedal pulses strongly when braking. The steering wheel and shifter also pulse during braking.

I know that I've got a problem with bent wheels. I also know that I need new rear brake pads. Could these things alone be causing my problem or can I be reasonably sure that my rotors are also bent?

I've got a good excuse here to upgrade to 16" wheels but I want to do the brakes at about the same time if I can be sure that I need to replace them. I want to be sure though so that I don't fix a problem that isn't there.

I should also add that I've never done brakes before but it seems simple enough. Any tips for the first timer that anybody wants to add would be appreciated. Specifically, I want to know before I start, what exactly I need to get besides the rotors and pads. Are there any special tools that I need? I've read about problems with squealing. Is there something that I can do to fix that while I've got everything apart? What about a hardware kit? What is included in this, do I need it?
