93 240 cold start problems 200 1993

I've seen a few posts regarding cold start problems, but my symptoms slightly different, which is why I'm posting this...

When I start the car ('93 245 67K) on cold mornings, it fires right up and runs smoothly for about 3-4 seconds, then for about 10 seconds after that it runs v-e-r-y roughly - sputters, shudders, smokes and nearly stalls. After the 10-seconds or rough running, it returns to normal and has no problems that I'm aware of. If I try to rev the engine during the 10 seconds, it will shudder more violently and produce more smoke, but this will also "clear" the rough idle. I typically just let it go away on it's own.

This morning - one of the coldest we've had in a while - it actually did stall during "the 10 seconds". I was finally able to start it by cranking for quite a while and depressing the gas pedal.

The rough idle never occurs when returning home at the end of the day - only in the morning, the colder it is the worse it is, and it seems to be more severe if I have left the car for a day or so rather than just overnight.

It doesn't seem like 13-14 seconds would be enough time for anything to warm up enough to make a difference, which is why I'm a bit confused. That, and my real-life, VW-Vanagon-induced fear of tinkering with fuel injection!
