Bet you re-used the flange nuts. Volvo's use self locking nuts on the pipe flange and manifold to head. They WILL loosen after a while if re-used. Always use NEW lock nuts for these repairs or you will be doing it again. I know everybody hates dealers, but Get a gasket and 3 new nuts and I bet your problem will go away.
