timing belt tensioner 700

1990 740 non-turbo,240,000 miles. replaced the timing belt and tensioner at 200,000 miles. The tensioner failed today,separating into two pieces and dumping all the ball bearings. The two pieces had not a trace of grease.The belt was damaged. I have changed the belt several times adjusting and then tightening the the tensioner nut each time to specs. What could have caused this to fail with so few miles. Did I forget to do something ? Could it have been misaligned? It did run fine for 40,000miles. What could I check or do before finishing the job,so this will not happen again.
