You're on the right track with those parts. I'm going to suggest replacing the rear trailing arm bushings, as they can give some unpredictable axle movement. While they're under there (that's really a job for a shop) get all the other arm bushings checked including the panhard rod.
Engine and trans mounts are also probably due for a change. Another distinct possibility is a rear wheel bearing. Not being able to drive and hear it, it's hard to judge but there's really not much else. Outer rear bearings are really easy and not expensive.
