240 transmission advice 200

Hello, I have a 1990 Volvo 240dl. I love my car. It has 310,000 miles on it. I think both the transmission and engine are the originals. Since I have had the car the transmission has had a problem where it will only shift into 2nd gear, when it gets to the appropriate speed, and when you let your foot all the way off the gas. Also in cold weather (anything below 60) when I drive the car stays in 1st gear until its warm, then it will switch. I have driven all the way to school in 1st gear many times. I have tried adjusting the kickdown cable. I adjusted it dramatically to both sides, and it doesnt help. It seem to have a problem with not snapping back well. Any advice is appreciated!!! Thanks!
