worn cam 1800 1972

I fear that I might need a new cam. My lifters are very noisy but when I checked the valve clearances I found them to be right on specs. Also the noisier the lifters have gotten over time the rougher the engine runs. The timing gears are begining to whine occasionally too. Also when I checked the clearances I did it when the lobes were farthest from the lifter. You would think that this would be where you would find the greatest clearance but instead there seems to be pockets of greater clearance as you rotate the cam.
I would like to know if am interpreting these symptoms correctly or if somone out there has a different idea. Also does changing the cam and lifters require removing the head?
