Rough idling at start and in reverse gear 900 1995

940 Non Turbo

Have been experiencing rough idling in the morning, and when shifting gear to reverse but only when engine is cold.
At traffic stop, engine RPM will drop intermittently and cause car to shake for a second or two before RPM moving back up.

Have changed distributor, distributor cap, rotor, cleaned the IAC but to no effect except that car has better power on acceleration.

I don't think is the engine or transmission mount because the RPM drops when the car shakes. My trusted mechanic told me it is the cylinder head and suggested that I either changed a rebuilt one of have those valves and cylinder milled.
What could be wrong?
