You're requesting it by registering. Look in the Technical archives here- I posted a scan of the Volvo High Mileage Club application a month ago. They send you the badge(s) for free once they check the vehicle's VIN. I've got to apply for my 200K now for my 86. The wife's 740 is at 197 so I think I'll wait for 200, and send that one in as well.
