Sounds like piston slap to me. Mine had it bad when I bought it with 241k km. We rebuilt the engine at just over 260K km because it had leaking seals and I couldn't stand the slap. Well, about 60 km after the rebuild, it returned! LOL. Not as bad as before, but it's still there. I try not to worry about it anymore. I just run synthetic oil and keep it clean. Your Vo has probably been doing that for at least 100K km....
