Is it the clutch or the gearbox? 700 1992

-92 740. B230F. M47. 200k km. Wery hard sometimes to get into 1:st gear. Always "grinds teeth" when putting in reverse. Makes me suspect the clutch more than the box. Clutch seems completely normal exept for above. Doesn't slip or seems to be very worn. Got no history though. Got the car 1 year ago and it has been the same way all of that time. Could it be the hydraulic system? Is that adjustable? Perhaps the clutch is just not fully disengaging?
