What's eating my timing belts? 200 1985

I just had a look at the timing belt the other day. The leading edge of it was worn down to the point of seeing the metal bands in the belt. Yuck.

So I slapped on another one, this time careful to make sure it was pushed towards the rear of the pulleys. Same thing, it just rode forward. So I took everything off, took a look at the tensioner (my first guess) and everything looked okay. However, I did notice that the tensioner isn't quite flush with the head. So checked it again, and for the life of me I can't keep the tensioner flush with the head. So I'm thinking maybe this is normal.

The oddest thing is that the belt isn't rubbing against the cam sprocket's front cover. In fact it doesn't appear to be rubbing against anything.

Could the hole in the head be misshapened (doesn't really look like it to me)?

I seem to have misplaced my big ol' Channel Loks so I decided against replacing the tensioner bearing.
