Serpentine belt mystery 900

Ok fellow bricksters i am stumped, on somthing that is realitively basic. 92 960 wagon. Belt partially shredded. After replacing belt I see that the back edge of the belt as it comes on to the alternator pully is working its way to the back and over the back lip of the pulley. Continues to do the same thing on the PS pully, from tensioner pully it straightens out and fits perfectly on the remaining surfaces till it reaches the alternator. Driving it home, following my wife, she pulls over. I check it, the belt has completely turned itself inside out and has placed itself on the other side of the tensioner pulley. Removed belt, turned right side out and reinstalled it, drove 10 more miles and made it home. Could this be caused by a weak tensioner. All pullys turn with ease, nothing has any slop in it. Any ideas??
