Clutch Shudder when Taking Off - Time for New Clutch? 200 1981

My 1981 242 B21F is starting to shudder when I accellerate from a stopped position, and, to a much lesser degree, when I shift gears. The shuddering can easily be felt by everyone in the car. Could this be caused by:

Bad/Unbalanced Clutch?
Clutch Cable?
U-Joints/Driveshaft?
New motor mounts combined with old transmission mounts?

How can I diagnose where the problem is before I drop tons of money on the clutch?

Thanks a lot,
Adam
