clutch started slipping suddenly 200 1979

My friend wants me to fix the clutch in his 1979 240 wagon with a 4 speed with a non-working OD. It started slipping suddenly-- I thought that clutches went out by progressively getting worse and worse, not just slipping suddenly, so I suspected perhaps an oil leak getting on the clutch disc or something, but I don't really know. Can a clutch just fail one day? It'll drive, but it slips in first and second gear unless you accelerate really carefully, and probably slips in 3rd and 4th too. I checked the clutch adjustment, but it was adjusted all the way out. Also, the protective boot covering the clutch lever was not fully inserted, causing a small area where dirt could get in. When I peered in there with a flashlight it looked dry. If it is a worn out clutch disc, what is involved in replacing it?
