Real Rough Running 700 1991

740 turbo, barely starts, I have to put my foot into it to keep it running but even then it is real work to keep it moving forward. Once it is at highway speeds there is no problem, but I know I am burning lots more fuel than I should (mileage is now well below 20 miles per gallon) I have heard possibilities from the 02 sensor to distributor problems. The car has been running rough for some time (6 months) and has recently reached that point that something has to be done. I have replaced plugs, wires, fuel filter. Where to go next?
